Please complete this form if you are a keyworker or have a child with an EHCP or social worker and you woud like them to start school from 5th January.

Please note that if you are a keyworker the Trust have decided that school places will be available where

either 

you are a single parent, working in a critcal area

or

you are a two parent family and BOTH parents are working in a critical area

If you do meet the critera above please can you email the school with your ID card(s) or a letter from your employer confirming your job is critical to the COVID-19 response. Thank you
